Vaccinations

​

Gold Stat Pediatrics values each family's personal decision on the vaccination of their children. Dr. Patel does encourage vaccinations. We do not discriminate on Vaccination Status of any child, parent/guardian, or family member. 

​

You may be asked to sign a waiver acknowledging you were informed of vaccination requirements and your preference. 

Late Show to Appointments

​

If you are more than 15 minutes late to your appointment we reserve the right to re-schedule your appointment. We understand there is traffic and things happen, please call us ahead of time. If you get more than 15 minutes late we do reserve the right to re-schedule your appointment. ​

​

If you are late more than 30 minutes it is considered a No-Show and we will re-schedule your appointment. 

​

If you cancel your appointment less than 12 hours in advance, it will be considered a "No Show."

 

No Show to Appointments

​

If you do not show up to your appointment or are more than 30 minutes late to an appointment, we will deem this a "No Show." 

​

If you cancel your appointment 12 hours before your appointment time, it will be considered a "No Show." 

​

Patients who have more than 2 "No Show" appointments over 2 years could be discharged from the practice. Of course, there are special circumstances and we ask for your clear and honest communication. 

 

Male Circumcision â€‹

​

Male circumcision is a personal choice of each family and Gold Star Pediatrics does not discriminate against families choosing their preference. Gold Star Pediatrics has a "zero" pressure policy where we do not pressure parents/guardians into making medical choices for their children. 

​

Dr. Patel does perform circumcision only if the family wants one and signs an explicit consent form. Dr. Patel only performs circumcision in his Gold Star Pediatrics North Port office.

Ear Piercings â€‹

​

Dr. Patel can provide ear piercings for children. 

​

Payment

All co-payment, deductibles, and other costs are due at the time of service. If there is no payment provided, your non-urgent appointment will be rescheduled for another time. Patients are responsible to adhering to Financial Policies in place. There are no payment plans for non-urgent office visits. 

​

Payment will be due in full if we do not participate with your insurance plan or are unable to verify your insurance coverage at the time of service or if your child is not yet added to your insurance.

​

Newborns

Newborns have an additional financial policy parents must sign before services are rendered. Payment will be due in full, subject to the fee schedule provided to parents, if we do not participate with your insurance plan or are unable to verify your insurance coverage or if your child is not yet added to your insurance.

ZERO PRESSURE POLICY​

​

Gold Star Pediatrics has a "zero" pressure policy where we do not pressure parents/guardians into making

medical choices for their children.

 

Gold Star Pediatrics strives to provide quality care that is tailored to each individual family and child. Gold Star Pediatrics will inform each parent/guardian of their children's health progress, any concerns, positive strides, areas of focus, any missing immunization requirements for schools/state compliance, and provide evidence based medical advise as per American Academy of Pediatrics. 

 

We respect each parent's and guardian's choice regarding their children. You may be asked to sign a waiver acknowledging you were informed of vaccination requirements and your preference.
